The employee evaluation form is a valuable tool to assist employers
with employee performance management.

A mark for unacceptable or poor performance on the employee evaluation form must be accompanied by a comment and the assessment must be done by the manager or supervisor of the employee.

Using an Employee Evaluation Form for Ongoing Assessment

Previous employee assessment forms can be used as comparison to note improvement, or the lack thereof in problem areas. The employee evaluation form can also be used to determine and justify increases, performance bonuses and incentives.
